Do you need to pay an excess after Worldline's intervention due to credit card fraud?  Fill in this online claim form to appeal to your insurance regarding the fraud.

Are you ill or do you need to cancel your trip? In this case, please send two additional documents with the claim form: the medical certificate (to be completed by the doctor) and the standard medical certificate for illness and the authorisation for the management of personal medical data. 
Attention: send the claim form to the relevant department (which you can find on the form) within 8 calendar days after the date of the incident.

Are you ill or do you need to cancel your trip? If so, please also send the following two additional documents along with the claim form:

  1. The medical certificate (to be completed by your doctor) and the standard medical illness certificate
  2. The consent form for the processing of personal medical data

Important: Please send the completed claim form to the relevant service (as specified on the form) within eight calendar days of the incident date.
