Do you have Top Dog & Cat insurance and has your pet suffered:
- An accident?
Trauma, injury, impact, fracture, bite, ingestion of an object…
- An illness?
Infection, tumour, condition requiring care, treatment or surgery.
Declaring or managing a claim
Follow these steps, depending on your situation:
1.To make a new claim :

- Ask your veterinarian to complete and sign this report form. You will need to attach it to your online claim declaration. This form is not required for costs related to prevention (e.g. vaccination, antiparasitic treatment) or for costs resulting from death (e.g. euthanasia, cremation or burial).
- Open a new file online using the unique link below. Then select 'Open a new file'. Complete the requested information about your pet's situation and upload the supporting documents.
Important: Please have your Top Dog & Cat insurance policy number ready for the policy under which you wish to make the claim.
2.To add costs to an existing file :

- Select 'Add costs to an existing file'.
- Complete the requested information about your pet's situation and upload the supporting documents.
Important: please have your pet's Top Dog & Cat file number ready.

For all non-urgent claims, please use our online reporting form: via the online form in Easy Banking App (Communication > Help and Contact > Report a claim) or Easy Banking Web.
